Provision of Anonymously Processed Information to Third Parties

Pursuant to laws and regulations, Eisai produces or acquires the anonymously processed information with an appropriate method so as neither to be able to identify a specific individual nor to be able to restore the personal information, and then provides such anonymously processed information to third parties by explicitly stating in advance to the third parties that the information provided is Anonymously Processed Information. Furthermore, Eisai takes necessary and appropriate actions for prevention of leakage of, loss of, damage of, as well as for security control of anonymously processed information.

The categories of information related to individuals contained in anonymously processed information provided by Eisai to third parties and the methods of provision of such anonymously processed information are as follows:

In case that Eisai produces the anonymously processed information and provides it to a third party

1. Purpose

Provision of the safety information of Eisai’s pharmaceuticals to patients and medical professionals, creation and research of statistical information, and development of products and services.

2. Categories of information relating to an individual contained in the anonymously processed information

Date of birth, gender, height, weight, age, nationality, and information related to diagnosis and treatment (including, without limitation, information related to disease, medicine, examination, and history of admission and discharge of a patient from the hospital) , and information related to other health conditions.

registration information for the services*, timeline, family doctor / primary doctor, hospital visit records, healthcare information, device information of the service user, information when making inquiries, service usage history

3. Methods for providing anonymously processed information

      • Upload to the server;

      • Transmission via email; or

      • Provision via external storage media such as DVDs.

In case that Eisai acquires the anonymously processed information and provides it to a third party

Preparation of the statistic information and other information, and development of products.

2. Categories of information relating to an individual contained in the anonymously processed information

Date of birth, gender, and information related to diagnosis and treatment (including, without limitation, information related to disease, medicine, examination, and history of admission and discharge of a patient from the hospital)
